Here in Lanarkshire we have our very own falls Hub.
This small, experienced team has been set up to improve the care for people in Lanarkshire who are worried about falling or who have been assessed as having a falls risk.
Staff in the team will carry out a telephone falls risk assessment and will support the person and/or their carers to make the best use of the specialist information and services available in their local community.
