PREVENT is a referral-only programme designed to support patients living with type 2 diabetes or pre-diabetes to take control of their health.
Primary care colleagues can refer eligible patients via SCI Gateway to the NHS Lanarkshire Healthy Lifestyle and Weight Management Service (LWMS). Patients referred from secondary care may also be directed into PREVENT if they meet the criteria.
The programme runs over 15 weeks and focuses on helping patients better manage their weight and blood sugar levels.
PREVENT is delivered by specialist LWMS clinicians alongside trained health instructors, offering a supportive and structured approach.
Patients can expect support with:
- Diabetes education (excluding CGM, insulin management and carbohydrate counting)
- Making sustainable lifestyle changes
- Achieving and maintaining a healthy weight
- Building confidence with physical activity
- Follow-up HbA1c testing and a remote clinical review at 15 weeks, with an optional review at six months
The programme is available at venues across North and South Lanarkshire, with sessions running on different days and times to suit patients’ schedules.
Corinne Payne, Clinical Nurse Specialist – LWMS, NHS Lanarkshire, said: “We launched PREVENT after recognising that an intervention was required to offer specific support for people living with pre-diabetes or type 2 diabetes, to either reverse the condition or reduce the risk of problems.
“Outcomes so far have been really positive, with participants achieving meaningful reductions in HbA1c (average blood sugar levels) and an average weight loss of over five per cent. Feedback has also been encouraging, with high levels of patient satisfaction and strong attendance throughout.”
