Our frailty strategy was the centre of focus at a busy event last week.
The meeting, the NHS Lanarkshire Strategy for Preventing and Managing Frailty: One Year On, had two main purposes, explained Hazel Gilmour, NMAPH Frailty Consultant.
Hazel said: “The first was to pause and reflect on the first year of collaborative work that has taken place through the strategy. New connections have been made between teams and individuals as we push against silo working and take forward the aims of the strategy, which focus on a proactive, person-centred approach.
“The second purpose was to consider how we move forward, both in relation to preventative medicine and ageing well as well as how we support Lanarkshire residents already living with frailty to live their best lives.
“The event allowed stakeholders to pledge contributions to the next steps, which will feed into three main workstreams: promoting health and wellbeing; tackling inequalities and delivering sustainable healthcare.”
Topics covered at the meeting at North Lanarkshire Council’s Social Work Department in Cumbernauld, included:
- Active wards;
- Blantyre Life;
- Carers Academy;
- Frailty in acute;
- Strategy for Prevention and Managing Frailty Implementation Group.
Dr Helen McKee, Associate Medical Director for Frailty, added: “A group of excellent speakers from the Interface Directorate, AHP teams, Social Work, Medicine of the Elderly and Carers Academy were the catalyst for rich conversations around pathways and models we’d like to grow.
“True to the frailty strategy, the focus of discussions centred on a proactive, person-centred approach and how we upscale good practice.”
