Community and voluntary services show their support for person-centred care

University Hospital Hairmyres Allied Health Professions networking in-service demonstrates partnership working for patient-centred care.

The Health Promoting Health Service at University Hospital Hairmyres co-ordinated a networking event on 28 February 2024 within the hospital restaurant.

This event was hosted for community and acute allied health professionals, with attendance from social work, clinical support workers, student nurses and junior doctors. The event benefitted from the support of 20 of our community and third-sector partners to support ‘What Matters to You’ conversations. In attendance on the day were Improving the cancer journey, long-term conditions management, information on lifestyle and health improvement, money/benefits, employability, home energy Scotland and Lanarkshire carers centre amongst others.

Angela Donaldson, Health Promoting Health Service lead at University Hospital Hairmyres said “Conversations about the wider determinants of health are important when providing person-centred care. When a person accesses secondary care there are often social considerations to be aware of during and after this journey.

“Often, social and economic issues are a trigger for accessing care in the first place. The Health Promoting Health Service support staff and patients with non-medical issues that can be a catalyst to accessing medical care, and to support discharge planning and recovery”.
