The Lanarkshire-wide Tissue Viability Service team has been celebrated for their dedication to delivering innovative, patient-centered wound care that truly impacts patients’ lives. Specializing in wound prevention, assessment, and management, the service ensures expert care that promotes healing and prevents complications.
Since February 2020, the team has transformed care with the “Near Me” virtual wound assessment service, enabling Tissue Viability Nurse Specialists to remotely assess wounds across care homes, hospitals, and treatment rooms in Lanarkshire. Through video calls, the team provides timely and expert wound care without requiring travel, ensuring patients and families receive holistic and supportive assessments wherever they are.
Since its inception, the service has delivered over 2,500 virtual assessments, increasing the proportion of patients receiving visual assessments from 41% in 2020 to 96% today. An impressive 86% of nurses rate the “Near Me” assessments as excellent or good, while 93.5% of patients rate their experience positively. One family member shared, “My father was seen in his home, and I could join using my iPad, allowing me to be part of the appointment to help heal his wound.”
