Launching NHS Lanarkshire’s ‘Reporting A Discriminatory Incident’ (RADI) Site

Oct 1, 2025

Our new NHS Lanarkshire’s confidential platform to report a discriminatory incident, has launched today (Wednesday, 1 October). Reporting A Discriminatory Incident (RADI) has been created to support NHS Lanarkshire & HSCP staff, students and contractors, to safely and securely report any incidents of discrimination in the workplace.

Reports can be submitted anonymously or with your contact details, depending on your preference.

You can access the RADI reporting form in the following ways:

  • Via direct links on the Staff Support page under Reporting A Discriminatory Incident
  • the Equality and Diversity page

The reporting site will go live today (1 October 2025).

Jacqui Jones, Director of HR commented: “Every member of our workforce deserves to feel respected, valued, and heard. The RADI system reflects our continued commitment to fostering an inclusive, supportive environment where discrimination of any kind is not tolerated—and where concerns are addressed with transparency, fairness, and compassion.

“We strongly encourage all staff to use this platform if they experience or witness discriminatory behaviour. Your voice matters. By speaking up, you help shape a healthier, more equitable NHS for everyone.

“Thank you for helping us lead this change.”
