The IBD nursing and pharmacist team at Wishaw Hospital is receiving a Big Shout Out in recognition of the quality, compassion and teamwork they bring to delivering outstanding patient-centred care.
The team demonstrates the very best of multidisciplinary working, bringing together the expertise of gastroenterologists, IBD nurses, dietitians, pharmacists, psychologists, surgeons and administrative colleagues to ensure patients receive coordinated, timely and holistic support throughout their inflammatory bowel disease journey.
Working closely together, the team ensures that care is joined up and responsive to individual needs. Weekly multidisciplinary team meetings provide an opportunity to discuss complex cases in real time, enabling shared decision making and ensuring care plans reflect the whole person, not just their condition. Their commitment to communication, innovation and continuous improvement helps ensure that care remains evidence-based, up to date and focused on achieving the best possible outcomes for patients.
One example of this approach can be seen in the urgent care of a young patient with severe ulcerative colitis. Through close collaboration, the IBD nurse and pharmacist were able to coordinate rapid access to biologic therapy while also providing reassurance, clear information and ongoing support to the patient and their family during a particularly difficult time.
As a result of this swift and compassionate response, the patient made a strong recovery and has been able to return to everyday activities, supported by continued follow-up from the team to help maintain their quality of life.
