About Palliative Care
Palliative and End of Life Care are integral aspects of the care delivered to those living with and dying from any advanced, progressive or incurable condition.  Palliative Care is not just about care in the last months, days and hours of a person’s life, but ensuring quality of life for both patients and families at every stage of the disease process, from diagnosis onwards.  Palliative Care focuses on the person, not the disease, and applies a holistic approach to meeting the physical, practical, functional, social, emotional and spiritual needs of patients and carers.
Palliative Care in Lanarkshire
In Lanarkshire we aim to meet palliative care needs and provide services in the setting of patient/carer choice, wherever possible.  For many people this will be in their own homes, or their care home of residence. Admission to an inpatient bed will only happen where absolutely necessary, with discharge facilitated as quickly as possible. Specialist palliative care is available for people with more complex needs, but most palliative care is delivered by other teams.

- Compassionate Lanarkshire (CLAN) is a partnership between St Andrew’s, Kilbryde and Strathcarron (call 01236 772004) offering complementary therapies and befriending to people and families affected by life limiting illnesses

- Our new community specialist palliative care referral and advice line should be used for all community referrals. It is staffed Monday to Friday 9am to 5pm by a senior nurse and a Consultant in Palliative Medicine. Old printed referral forms should be disposed of.
