Integrated Rehabilitation Teams (IRTs)
There are 6 Integrated Rehabilitation Teams (IRTS) in North Lanarkshire who provide adults with health and social care needs support, advice and rehabilitation within the home environment.
The teams bring together health and social work and include physiotherapists, occupational therapists and support assistants. They work closely with hospitals, emergency departments (ED), GPs, social work and the voluntary sector.
The IRTs can provide rehabilitation in the community while identifying and providing any additional supports e.g. equipment and adaptations or home care.
How to refer
Your GP, Social Worker or Healthcare Professional can refer you to the service.
