Discharge without Delay:
- Lanarkshire has been one of four national ‘pathfinder sites’ for the DwD programme. That means work being pioneered locally is being used to inform and shape the now nationwide approach to help prevent delays and reduce length of stay in hospital.
- This short animated clip provides a clear overview of what the DwD programme is, why we are doing it and how it is based on a rich tradition of partnership working.
- The following animation has been launched to highlight the support available to implement the DwD programme.
Planned Date of Discharge
- PDD aims to establish a clear date of when someone will go home (or to a community setting) – at the earliest possible stage during their stay in hospital. By providing a clear route-map through the hospital ‘journey’, PDD is geared to reducing any uncertainty and anxiety. Watch our animation here:
