Chronic Disease Monitoring

Changes to how NHS Lanarkshire helps you manage your chronic disease / long term condition

The way we monitor your condition(s) is changing – this may be for diabetes, high blood pressure, kidney disease or another condition. This is due to the new national GP contract, which is happening across all Scottish health boards. Part of the new contract is moving chronic disease monitoring away from GP practices to being provided by NHS Lanarkshire in treatment rooms.

This means that you will no longer attend your GP practice for chronic disease monitoring, but will continue to get all the advice you need to manage your chronic disease from your GP practice.

What is the difference between chronic disease monitoring and management?

Chronic disease monitoring – many people already attend the treatment room for bloods. Under the new changes the nurse will also carry out other observations to provide information to help monitor your condition. These may, when appropriate, include blood pressure, height, weight and urine analysis. This information is then sent to your GP practice.

Chronic disease management – will continue to be provided by your GP practice. This will include any discussions on the results of the observations carried out at the treatment room and how you live with your condition.

What are the changes?

Traditionally, chronic disease monitoring and chronic disease management are carried out at GP practices. These changes see the monitoring part move away from GP practices to treatment rooms, run by NHS Lanarkshire. However, depending on where you live, your chronic disease monitoring may remain at your GP practice.

This new way of working has been developed to comply with the 2018 GMS contract. These changes are happening across Scotland.

Your GP practice will be in touch with details about how to make a chronic disease monitoring appointment.

You don’t need to do anything until you are contacted by your GP practice.
