Keep Well
Keep Well is a Nurse led service which works in partnership with a variety of services to deliver health checks across Lanarkshire in community settings.
The team engage with people whom experience health inequalities and are at more risk of poorer physical health and mental wellbeing.
Individuals from targeted groups aged 16 years and over are offered support with health behaviour change, goal setting and lifestyle modifications in order to improve health outcomes.
